Contact Details

Thank you for making contact with us.

So that your feedback is dealt with by the most appropriate person, please select the email address that has been provided for your organisation.

We are encouraging feedback in relation to the content of the formulary / PAD. For example, please tell us if you think that there is a drug missing from the formulary that should have been included, or you have noticed a potential error.

Perhaps there is a commonly used search term that is not producing any search results? Let us know so that we can build more search terms into the system.

We also welcome feedback that will help us improve future versions of the platform.

Important - this contact function must not be used for raising clinical enquiries. Please make contact with your Pharmacy or Medicines Optimisation Team in the usual way for raising clinical enquiries.

Thank you
