Freestyle Libre 2 - discontinued
The Surrey Heartlands Joint Formulary has been updated to remove the Freestyle Libre 2 sensors.
Freestyle Libre 2 Plus sensors were added to the formulary in July 2024 and are one of the preferred CGM devices for Type 1 and Type 2 diabetics.
The supply of Freestyle Libre 2 sensors is being phased-out from August 2025 and prescriptions for these will not be accepted for reimbursement by NHS Prescription Services from October 2025.
All patients receiving prescriptions for Freestyle Libre 2 sensors should be switched to an alternative device. Freestyle Libre 2 Plus or Dexcom ONE Plus are suitable alternatives.
