The specialist team will initiate treatment and provide at least 3 months supply before any request is made to transfer prescribing responsibility to Primary Care.

The Surrey & Sussex Integrated Care Board Area Prescribing Committee (APC) approved a BLUE formulary status for sodium zirconium cyclosilicate (Lokelma®) for persistent hyperkalaemia in people with CKD stage 3b to 5 or heart failure, noting that its use for acute life-threatening hyperkalaemia remains as RED.
The specialist team will initiate treatment and provide at least 3 months supply before any request is made to transfer prescribing responsibility to Primary Care.
The Area Prescribing Committee recommends sodium zirconium cyclosilicate as a treatment option for the treatment of hyperkalaemia in line with NICE TA599
Sodium zirconium cyclosilicate will be considered RED on the traffic light system
Primary care prescribers should ensure that patient medication records include any medicine for which prescribing remains the responsibility of secondary or tertiary care. This will ensure that GP records, which are accessed by other healthcare providers, are a true and accurate reflection of the patient’s medication.
