Welcome to the Surrey Heartlands Joint Formulary & Prescribing Advisory Database (PAD)

Please take a moment to read the welcome article. Levemir (insulin detemir) Penfill and FlexPen discontinuation

Levemir Penfill and FlexPen are to be discontinued. Stock likely to be exhausted by December 2026. All patients will need to be switched to an alternative insulin - see article for more information and advice.

Semaglutide TABLETS - CHANGE OF STRENGTH

Semaglutide tablets (Rybelsus) are now available in new, lower strengths. Patients will need to be switched to a new strength and will require appropriate counselling to avoid the risk of incorrect dosing. The 3mg, 7mg and 14mg tablets are being phased out and being replaced by 1.5mg, 4mg and 9mg tablets. See the full article for more details and resources.
